  1. Unfortunately I either 1) didn’t get a receipt, or lost it on the way home.
    I purchased two meals from the 12th ave. Jacks in Nampa Idaho. Both were the Teriyaki Chicken Bowel, 1 combo and 1 bowel, with an order of egg roll on the side and a small coke, no ice.

    After I got home I discovered that the bowels were filled only 1/2 way with product. This was extremely disappointing. I have ordered this same meal many times at this same restaurant (though not lately). I would guess that these meals were about 40% lighter than ones served in the past.

    The quality of the food WAS very good. The quantity was #10. It’s to bad really. I expected better.

    The only thing i’m seeking is an answer to my question, Is this what I should expect in the future? A simple yes or no will suffice.

I am a long-time customer of your restaurant and a senior. I look for a bargain wherever I can find one and I try to utilize every jack in the box coupon that I see in our local paper.   3. The receipt has 14 numbers. The numbers are arranged in sets of three digits each, except for the last set which has only two (Total of 5 sets of numbers). However, the only survey site I can reach has room for only 4 sets of numbers with the last box smaller than the other 3. This means the receipt should have 3 sets of 4 digits each and one set of 3? But, it does not. It is impossible to fill out the survey. Is this by design? It did not use to be this way. Can you send a link that I can use to imput the 14 digits from my receipt?

    So disappointed. On my way to work yesterday, on Mothers Day pulled into your facility on Fremont Blvd, Fremont CA, Irvington district. Ordered a ultimate cheese burger using a coupon, drove to the window in like 30 seconds and my order was ready. I said that’s my order? Her reply was yes. I said how can that be and she responded, it’s fast food! I said it better not be old and she giggled. I drove to work, only 3 minutes away and opened up my food and sure enough, my food was cold and the meat looked old. The cheese was not even melted. There was not any other customers around, parking lot was empty. Fries were not warm either. If I had time I would have went back but had to be at my job.
    I do not do this very often, but felt you should know what is going on so that you can fix the issue.

Left the ER after being treated for a fractured rib, stopped at your location #898 on 9/23/21 @ 2:48am. Ordered a jumbo jack with cheese and a small coke. Got home tired and sore opened my bag and I have a chicken sandwich. I was extremely disappointed. I don’t normally leave comments but again, I was so disappointed with this experience.
